Arizona uses “assisted living home” for an assisted living facility that provides resident rooms to ten or fewer residents. Confirm the authorized service level and scope before preparing an application.

Eleven or more residents

Arizona uses “assisted living center” for facilities serving eleven or more residents. Building, staffing, documentation, and inspection preparation must match the applicable rules.

1. Define the license and services

Confirm whether the property will operate as a home or center and whether it will provide supervisory, personal, or directed care services.

2. Prepare the property and records

Review the applicable ADHS checklist, safety requirements, policies, sample forms, personnel records, proposed staffing schedule, and scope of services before requesting inspection.

3. Submit through ADHS

Arizona requires licensing applications and renewals through its electronic Licensing Management System. Keep copies of submissions, receipts, and agency correspondence.
